    New patients can download and complete the form before their first appointment. Existing patients may log in through the portal for scheduling, secure messaging, and test results.

    Please bring:

    • Valid photo ID
    • Insurance card(s)
    • Referral or authorization (if required)
    • List of current medications and dosages
    • Relevant medical records, imaging reports, and scans

    If you are being evaluated for memory or cognitive concerns, please consider bringing a family member or caregiver who can assist with background information.
    We suggest arriving 15 minutes early to complete required medical history and HIPAA forms.

    We accept most major insurance plans. We always recommend verifying coverage with your insurance provider and our office before the visit.

    Please have the following available when you call:

    • Referral details (if required by your insurance)
    • The reason for your visit (consultation, EEG, EMG, etc.)
    • Insurance information
    • Basic personal details such as name, address, and a daytime phone number

    If the visit is related to a motor vehicle accident, you may also need to provide claim details and insurance adjuster information.
